The Patient Access Network (PAN) Foundation is hosting a survey about patient assistance. The PAN Foundation is a national nonprofit organization that helps people living with life-threatening, chronic and rare diseases pay for the out-of-pocket costs of their prescription medications. Prevent Blindness has been working with PAN for some time to provide patients with education and support about their conditions.
PAN is interested in learning more about the financial assistance patients may need to cover the out-of-pocket costs for their medications as well as the challenges patients face when looking for financial assistance. Individual responses will be anonymous. All responses will be combined and analyzed together. The survey should only take about 5 minutes.
